菲律賓人應準備之文件DOCUMENTS REQUIRED FOR FILIPINO FIANCE OR FIANCEE

1. 護照. Present and Previous Passports. (Original & a photocopy of all pages used.)

2. 出生證明須經菲律賓外交部驗證，年逾十二歲始作登記之遲登出生證明，或文件修正處超過三項以上者，須完成法院身分判定，並提供足資採信之身分證明文件。年逾二十一歲遲登者，得不予受理。Birth Certificate secured from the National Statistics Office, and duly authenticated by the Department of Foreign Affairs. Affix Documentary Stamp. (Original & a photocopy) *NOTE: Birth certificate registered after 12 years old, or document with Correction of entry exceeding 3 items must seek an affirmative court decision on the assumption of identity and submit enough identity for reconsideration. Late birth registration done after 21 years old would not be accepted.

3. 單身證明須經菲律賓外交部驗證. Certificate of no records of marriage (CENOMAR) secured from the National Statistics Office, and duly authenticated by the Department of Foreign Affairs. Affix Documentary Stamp. (Original & a photocopy)

4. 如在菲律賓有前婚姻紀錄須提供結婚紀錄證明、註銷證明及死亡If one of the parties have previous marriage in the Philippines, attach Advisory on Marriages, Marriage Certificate with remarks of annulment, Death Certificate (if spouse is dead) and Birth Certificate of children secured from the National Statistics Office, and duly authenticated by the Department of Foreign Affairs. Affix Documentary stamp. (Original & a photocopy.)

5. 最近三個月指定醫院之健康檢查須經菲律賓外交部驗證(指定醫院請參閱附件).Medical Certificate issued within the last three months from Accredited Hospitals and duly authenticated by the Department of Foreign Affairs. Affix Documentary Stamp (Original & a photocopy). (Accredited hospitals refer to attachment) 含HIV愛滋檢測

出生證明跟單身證明,多申請幾份,申請結婚許可會用到。

有些地區會要求台灣人在菲國的單身證明,也一併申請。
